St. Anthony of Padua Catholic School strives to maintain a high level of wellness among our students and staff.
Every student enrolled in a Catholic school in the state of Texas must be immunized against vaccine-preventable diseases caused by infectious agents in accordance with the immunization schedule adopted by the Texas Department of State Health Services. All immunization records MUST have a physician's signature to be in compliance with the state--a copy of their immunizations from their patient portal is not acceptable (unless it includes a physician's signature). A student who fails to present the required evidence shall not be accepted for enrollment. The only exception to the requirement is a medical exemption signed by a licensed physician (M.D. or D.O.) authorized to practice in the State of Texas.
Vision and hearing screenings are conducted annually as mandated by the State of Texas.
Forms that should be completed for students with special medical conditions are listed below. A medication permission form can be used for PRN or daily medications (such as ADHD medications). If the student with asthma is older or in after-school sports the parent should have both the “Asthma Action Plan” and the “Asthma action plan for self-carry” forms completed.
